TO INITIATE THE NEW PATIENT PROCESS,
PLEASE FOLLOW THESE SIMPLE STEPS

2) Print out the forms

3) Fill in the form by hand

4) Please ensure the filled-in form reaches the clinic during working hours

5) The clinic will inform you if you are eligible to be a new patient and will fix an appointment for you with the doctor.

HOW TO REQUEST MEDICAL RECORDS

1. Complete HIPAA Privacy Authorization Form

2. Who May Request Medical Records:

 

You, if you are patient or the parent or guardian of the patient whose records are being requested.

 

If you are not the patient, parent, or guardian, then you must obtain written permission from the patient, sometimes using the form the provider gives you. Caregivers or advocates may be able to access records if the patient has provided written permission to the provider.

INSURANCE CLAIMS & PAYMENT POLICY

If we are a contracted provider with your insurance company, we will file claims on your behalf for your convenience; however your insurance company requires us to collect co-pays and/or other fees to meet your annual deductible. Payment is expected at the time services are rendered. We accept most major credit cards, personal checks, & cash. 

APPOINTMENT CANCELLATION POLICY

We strive to provide excellent medical care to you, your family and all of our patients. In order to do so effectively and efficiently, we have developed an appointment system that sets aside ample time for each patient.

"No-shows", and late cancellations inconvenience those individuals who need access to medical care in a timely manner. In an effort to reduce the number of such occurrences, we have implemented a Medical Appointment Cancellation Policy:

  1. We request you give our office a 24- hour notice in the event you need to reschedule your appointment. Our phone number is 888-599-6634.

  2. If you miss an appointment and do not contact us with at least a 24-hour prior notice there will be a cancellation fee. This applies to late cancellations and “no-shows.”

  3. If you are late for an appointment, you will be seen as soon as possible, though the office visit time may need to be shortened.

  4. Our office makes reminder calls for appointments. If you are registered for the patient portal, you will receive e-mail reminders as well.
